Five-Diamond Approach to Patient Safety Program

Program Overview

The National Five Diamond program is designed to assist dialysis facilities in improving both staff and patient awareness of specific patient safety areas.  There are 15 web-based educational modules addressing all aspects of patient safety.  All modules include objectives, required activities, tools and resources, and optional activities.  For each module successfully completed, the facility is awarded a “Diamond” culminating in special recognition for 5-Diamond facilities.

As of January 1, 2014 the program is entirely web-based, and can be accessed at  Users must register as either a “viewer” or “participant” and must indicate their Network affiliation (New York is Network 2).  Viewers may access modules, tools, and resources. Participants may access the site in its entirety.   After completing a module, participants complete and submit the required reporting form online; forms are reviewed and approved by the Network Manager; and participants can then print their own certificates.

Rewards for participants earning 5-Diamonds include:

•  Recognition on the 5-Diamond Patient Safety Program website
•  Recognition letter from the Network CEO
•  Five Diamond Plaque

This education program is free of charge and your facility may use any of these modules for educational programs.

If you officially enroll in the program by filling out an application your facility’s progress towards Five-Diamond status will be recorded and publicized.

The following schedule applies to Five-Diamond Status:
Achievement between 1/1/2012 and 12/31/2012 is valid until 12/31/2013.
Achievement between 1/1/2013 and 12/31/2013 is valid until 12/31/2014.
Achievement between 1/1/2014 and 12/31/2014 is valid until 12/31/2015.

Renewal of Five-Diamond Status
Renewal of status is only offered to facilities that earned/renewed Five-Diamond status during the previous program year.

Facilities wishing to renew their Five-Diamond status from the previous program year must repeat the mandatory Patient Safety Principles module each year their status is renewed and complete two additional modules of their choosing.  They may repeat modules completed in previous program years.
